Typhoid fever: the long fever
Typhoid is a bacterial infection from food and water handled without clean hands. The fever lasts. A single casual antibiotic, stopped on day three, is a common way to make it harder to treat.

In plain language
Fever that persists, headache, abdominal pain, and sometimes constipation or diarrhoea are the pattern. The person feels more ill than a ordinary cold. Blood culture, taken before antibiotics if possible, is the useful test. The Widal blood test is easy to misuse and is not a diagnosis on its own. Treatment is a full course of the right antibiotic, rest, and food. A perforated gut is the catastrophe, and severe abdominal pain is the clue.
Signs worth noticing
These are the clues families usually notice first. One of them, on its own, is not a diagnosis.
- Fever continuing beyond a few days without a obvious cold.
- Headache, loss of appetite, and abdominal discomfort.
- A rash of faint spots, which you may not see.
- Severe abdominal pain, vomiting, confusion, or bleeding.
What usually sits underneath
The germ invades from the gut into the blood. Contaminated water, cut fruit, and a cook who is a carrier are classic sources. Incomplete antibiotics breed resistance, which is now common in the region.
Care that is reasonable at home
- Drink water you trust. At home, that means water you have boiled or filtered properly, not a jug that has sat open.
- Wash hands before eating and after the toilet.
- If an antibiotic is started, finish the course unless a clinician stops it.
- Eat small meals. Do not ‘starve a fever’ for a week.
These steps are for someone who is otherwise stable. If the picture is changing quickly, skip the home trial and come in.
When waiting is a bad idea
- Fever that is high and you look toxic, or you cannot drink.
- Severe abdominal pain or a swollen silent belly.
- Confusion, a fit, or bleeding.

Questions people ask
Is the Widal test enough?
No. It causes both false reassurance and false labels. Culture and the clinical picture matter more.
Can I take last year’s strip?
No. The right drug and the full duration are the treatment. A leftover strip is how resistance happens.
When can I return to cooking for others?
When a clinician says you are no longer a risk. Do not cook for a gathering while febrile.
