The cough that does not leave

A cough of a few days with a cold is ordinary. A cough of three weeks is a reason to be examined. In India that sentence includes tuberculosis until someone has thought about it.

In plain language

After a virus, airway irritation can keep you coughing for several weeks, especially at night. Acid reflux, asthma, and post-nasal drip do the same. Tuberculosis, whooping cough, a foreign body, and some medicines (blood-pressure tablets of the ACE-inhibitor type) are the other classic causes. Coughing blood is never a watch-and-wait symptom.

Signs worth noticing

These are the clues families usually notice first. One of them, on its own, is not a diagnosis.

A dry night cough after a cold, in a person who is otherwise well.
Wheeze or heartburn with the cough.
Fever, weight loss, night sweats, or someone at home with tuberculosis.
Blood in the sputum.

What usually sits underneath

The airway stays twitchy after infection. Reflux sends tiny amounts of acid up at night. TB inflames the lung more slowly and is often accompanied by a general decline. Smoking makes every one of these worse and hides them.

Care that is reasonable at home

If the cough is clearly post-viral and you are well, give it a short time, not a month of random antibiotics.
Do not take a cough suppressant so you can keep smoking.
Note fever and weight. A kitchen scale is enough.
Come at three weeks, or sooner if you are unwell. Bring old chest reports.

These steps are for someone who is otherwise stable. If the picture is changing quickly, skip the home trial and come in.

When waiting is a bad idea

Coughing blood, even a little.
Breathlessness, chest pain, or a cough in a person with a weak immune system.
A child who is coughing until they vomit or whoop, or any cough in a very young baby that looks like hard work.

Questions people ask

Will an antibiotic stop it?

Not if the cause is a virus, reflux, or asthma. Antibiotics are for bacterial infections, which are the minority of lingering coughs.

Is TB likely if I feel fine?

Less likely, not impossible. Three weeks of cough is still worth a proper look.

Can reflux really cause a cough?

Yes, including in people who do not notice much heartburn.
